consent to the processing of personal data

I hereby, acting freely, of my own free will and in my own interests, and also confirming my legal capacity, providing information about myself, give consent to Limited Liability Company "Lada", INN 3827054167, address: 666137, Irkutsk region, Olkhonsky district, Khuzhir settlement, Taezhnaya, 30A (hereinafter referred to as the Operator) to process the following personal data of mine, specified by me both when booking Accommodation on the Operator's website and provided through my personal account on the Operator's website or in another way, for the purpose of processing and executing the booking of Accommodation, including the provision of additional services at the Operator's Accommodation Facility/Facility:

- last name, first name, patronymic (if any);

- contact information (phone number, email address);

- citizenship;

- gender;

- period of stay (arrival date, departure date, purpose of entry, room number);

- living conditions;

- registration address;

- information about guests;

- date of birth (day/month/year);

- identity document details (series, number, date of issue, department code, issuing authority, country of birth, region of birth, district, city, town).

To achieve the specified goal, the Operator may perform any actions (operations) or sets of actions (operations) performed using automation tools or without using such tools with the personal data specified above, including collection, recording, systematization, accumulation, storage, clarification (updating, modification), retrieval, use, transfer (provision, access), depersonalization, blocking, deletion, destruction of personal data.

I give consent to the Operator to transfer (provision, access) the personal data specified above to the following legal entity acting on behalf of the Operator: TRAVEL LINE SYSTEMS LLC, INN 1215180595, OGRN 1141215003214, legal address: 424031, Republic of Mari El, Yoshkar-Ola, st. Komsomolskaya, 112 for the purpose of automating business processes, centralized storage and management of the Operator data, as well as performing the following actions (operations) with them: collection, recording, systematization, accumulation, storage, clarification (updating, modification), extraction, use, transfer (provision, access), depersonalization, blocking, deletion, destruction.

I confirm that the personal data I have provided is accurate, current and reliable, and I also undertake to promptly inform the Operator of any changes in my personal data by email: ladahotel@bk.ru, or in writing to the address: 666137, Irkutsk region, Olkhonsky district, Khuzhir settlement, Taezhnaya, 30A.

This consent comes into force from the moment it is provided by checking the box on the Operator website.

The consent is valid until the purpose of processing is achieved or until the consent is revoked.

In accordance with the provisions of Part 2 of Art. 9 of the Federal Law of July 27, 2006 N 152-FZ "On Personal Data" I reserve the right to revoke this consent by sending such a revocation to the e-mail address: ladahotel@bk.ru, or in writing to the address: 666137, Irkutsk region, Olkhonsky district, Khuzhir settlement, Taezhnaya, 30A.

I have been warned that in case of revocation of consent to the processing of personal data, Operator has the right to continue processing personal data without consent if there are grounds specified in subparagraphs 2-11 of Part 1 of Article 6, Part 2 of Article 10 of the Federal Law of July 27, 2006 N 152-FZ "On Personal Data".
